What Is an Extraschedular Rating?

An extraschedular rating is assigned when a Veteran’s service-connected disability presents an exceptional or unusual disability picture that is not adequately captured by the standard VA Schedule for Rating Disabilities (VASRD). Under 38 CFR §3.321(b)(1), cases may be referred to the Director of Compensation Service for consideration of an extraschedular evaluation when the regular schedular criteria are inadequate.

When to Consider Extraschedular Referral

A referral may be warranted when the Veteran describes any of the following:

Marked Interference with Employment Disability causes significant impairment beyond what the assigned rating percentage contemplates (e.g., frequent missed work, inability to perform essential job functions).
Frequent Hospitalization The Veteran has required frequent periods of hospitalization related to the service-connected condition beyond what is typical for the assigned rating.
Other Exceptional Factors Symptoms, manifestations, or functional impairments not contemplated by the rating criteria (e.g., side effects of treatment, combined effects unique to the individual).

PCR Actions When Extraschedular Indicators Present

  1. Document the Veteran’s statements about how their condition affects employment, daily living, and hospitalization history in the case notes.
  2. Do not promise an extraschedular rating — explain that it requires referral and separate review.
  3. Note the indicators in the CRM activity record so the RVSR can identify the case for potential referral.
  4. Advise the Veteran to submit supporting evidence (employment records, hospitalization records, statements from employers or physicians).

“Based on what you’ve described about how your condition affects your employment, there may be an option for the rating specialist to consider additional factors beyond the standard schedule. I’ve documented your situation in your case file. Supporting documentation from your employer or physician would strengthen this consideration.”

Referral Process (For Awareness)

PCRs do not initiate referrals directly. The process is:

  1. RVSR identifies exceptional factors during rating
  2. RVSR prepares referral to Director, Compensation Service
  3. Director reviews and determines if extraschedular evaluation is warranted
  4. If granted, rating is adjusted outside the standard schedule
